Crouse Health’s Student/Observation Portal

Crouse Health welcomes thousands of students every year into our facility.
This site will guide you through the requirements you must complete to apply for a Crouse student experience.

High School Students: If you are interested in an observation experience, you must be a participant in our Choices Program.

College Students: Please follow the steps below to complete your application. Incomplete applications will not be accepted.

Applications must be submitted two weeks prior to the requested start date.

An affiliation agreement between Crouse Health and your school must be in place prior to the start of any student experience.

While we make every effort to place students within our facility, application for an experience does not guarantee acceptance.


Application Steps:

Step 1: Download the following three documents.

Step 2: Read the Orientation Manual.

Step 3: Complete the Hold Harmless and Release form.

Step 4: Complete the Health Clearance form and attach all necessary medical records to reflect vaccinations.

Step 5: If you will need access to our Electronic Medical Record during your student experience, download and complete:

Step 6: Complete and pay for Criminal Background check here:

Step 7: Once you have completed steps 1-6 go to the application page and submit.
